Your first visit
Most first visits are a dry eye evaluation or an eye exam. Both are unhurried, and you leave knowing what we found.
You can fill in the paperwork before you come, on the office's own forms page, or at the front desk. The message form on this website asks only for your name and how to reach you. Please keep your health details for the forms and for the doctor.

Before
We check your benefits and tell you what we find. If you pay yourself, you get a written estimate.
At the visit
Dariush takes measurements, and Dr. Cullen examines your eyes and explains what she finds.
What to bring
Your glasses, your contact lenses and cases, the drops and medicines you use, and your insurance cards.
